ELIGIBILITY & PARTICIPATION FORMS
Athletics eligibility and participation forms for the 2020-2021 school year will be completed online on Rank One at https://sequoyahschools.rankonesport.com. Rank One is active now for the 2020-2021 school year.
Parents/guardians who registered with Rank One last year will log in to your account to complete the forms for this year.
All new students and anyone who didn't set up an account last year will need to set up an account. You will be asked for a student ID number for your student during the early steps of the process. New students will receive a slip with their ID number with their acceptance letter. Returning students will use your lunch/cafeteria number. Returning students can find this four-digit student ID number on schedule printouts and on the parent portal. The forms that we need 1st are the Physical and Athletics Participant Form. These forms will be required prior to participation in any athletic activity. You will still use a paper form for the physical. The Athletics Participant Form (which includes concussion and sudden cardiac arrest acknowledgements) will be completed online. We will send a “Changing Schools” paper form for parent and student signatures later.
All other forms will be completed online. Please complete all eligibility/participation forms at your earliest opportunity, at least 1 week prior to the first scrimmage or game in which a student athlete wants to participate. New students will do all forms. These will be reviewed to determine the eligibility status [eligible for varsity, eligible for sub-varsity only (JV/freshman level), or not eligible at any level] for each student according to OSSAA rules. We will notify parents/guardians of students who are determined to be ineligible for varsity or ineligible at any level of their status and will discuss the process to become eligible. Parents/guardians can download your student's physical and other completed forms as needed while logged into your Rank One account.
Returning students are not required to do the Eligibility Questionnaire, but will need to complete all other forms each year.
If you try to set up an account, but don't receive an email from Rank One, check your junk email folder and spam email. If you still don't find an email from Rank One in junk or spam, you can still complete the forms without having an account. You can skip setting up an account by clicking the gray button that says “Continue as a guest" on the screen that opens when you start trying to fill out forms (see screenshot below). If you skip setting up an account, you won't be able to download completed forms on demand and will need to enter all information again next year instead of carrying some of it forward and confirming.
