COVID-19 Testing Consent Form

Please open and print the following consent form for COVID-19 testing on Monday, 10/19/2020 beginning with Freshmen students at SHS.

 COVID-19 testing consent form.

Parents or guardians may sign the form only. Students are not allowed to sign their own consent form regardless of age. Consent forms will also be available in person on student attendance days Monday-Thursday. Freshmen students may report to the old gym beginning at 7:45 am on Monday, October 19, 2020. Upper classmen will return on their day at with the same guidelines and schedule as the Freshmen.

We will follow the same attendance schedule as the initial orientation period.

Monday- Freshmen

Tuesday-Sophomores

Wednesday- Juniors

Thursday-Seniors

Friday- Virtual

Wear your mask, student ID badge, bring your device and we will see you Monday, class of 2024! Buses will run and more information will follow on residential availability in the near future.
